​​Appointment Information
In preparation for your visit please bring the following with you:

  • Driver’s license
  • Insurance card
  • Copay

We accept:

  • Cash
  • Visa, MasterCard, Discover
  • Check

Babson & Associates Primary Care P.C. is contracted with the majority of insurance companies.  If we are not contracted with a patient’s insurance we will still bill the insurance company as a COURTESY to the patient. Patients are asked to bring current insurance information to every visit. Copays are collected at the time of service. Uninsured new patients are asked to pay $125.00 toward the cost of their first visit at the time of service and make payment arrangements for the balance as well as any continuing treatment.
Additional information to bringing:

  • List of all medications and supplements that you are taking
  • List of current medical conditions and prior treating providers
  • Pharmacy phone number

We request 24 hours’ notice should you need to cancel or reschedule your appointment. If this is not provided, you may be charged a $25 fee. We understand that unforeseen events sometimes result in a missed appointment and these can be discussed with the staff of the practice.
All patients 18 years old or under must be accompanied by a parent or legal guardian.
Before your visit: Write down your questions and worries. If it’s a long list ask for a longer visit. If you see a specialist, ask them to send their report to your primary provider. Put your list or place your medicine in a bag and bring it with you to your visit.
In Your Provider’s Office: Relax! Ask questions! Take notes. Tell your provider if you don’t understand. Ask your provider the values of your last blood pressure, weight and lab test. Keep a record of these values. Ask when you should schedule a follow up appointment. Let the provider know if you need clarification on what was reviewed at the visit.
