Information for New Patients

Appointment Information

 
In preparation for your visit please bring the following with you: Driver’s license; Insurance card and your Copay

We accept: Cash; Credit Card (Visa, MasterCard, Discover); Check. Please setup an new account with our patient portal and complete the self-check in prior to arriving to the clinic 30 minutes early for your appointment.

Babson & Associates Primary Care P.C. is contracted with the majority of insurance companies.  If we are not contracted with a patient’s insurance we will still bill the insurance company as a COURTESY to the patient. Patients are asked to bring current insurance information to every visit. Copays are collected at the time of service. Uninsured new patients are asked to pay $125.00 toward the cost of their first visit at the time of service and make payment arrangements for the balance as well as any continuing treatment.

Additional information to bringing: List of all medications and supplements that you are taking; list of current medical conditions, prior treating providers; and your preferred Pharmacy.

We request 24 hours notice should you need to cancel or reschedule your appointment. If this is not provided, you may be charged a $25 fee. We understand that unforeseen events sometimes result in a missed appointment and these can be discussed with the staff of the practice. 


All patients under the age of 18 years old must be accompanied by a parent or legal guardian.


Before your visit: Write down your questions and worries. If it’s a long list ask for a longer visit. If you see a specialist, ask them to send their report to your primary provider. Put your list or place your medicine in a bag and bring it with you to your visit.

In Your Provider’s Office: Relax! Ask questions! Take notes. Tell your provider if you don’t understand. Ask your provider the values of your last blood pressure, weight and lab test. Keep a record of these values. Ask when you should schedule a follow up appointment. Let the provider know if you need clarification on what was reviewed at the visit.

Babson and Associates Primary Care serving patients throughout Wyoming, across the western plains of Nebraska, and the front range of the Rocky Mountains for over 45 years. We offer comprehensive medical care for the entire family. Other services include occupational physicals including FAA medical exams and DOT medical exams.

No News is Not Good News - No News is No News


In healthcare, the saying "no news is good news" is a dangerous misconception because no communication about medical test results can indicate a missed diagnosis, a lost specimen, or an oversight in the lab or office. Patients should always follow up to request and understand their results, as they have a right to their health data and should not assume silence means everything is fine, as this can lead to significant risks. 


You will be notified of any testing ordered by our office. If you have not heard from our clinic by 72 hours, please give us a call or message us through the portal.
